When you have completed the form please email to us at gram.hamiltonclinical@nhs.scot

Once we have accepted your registration request we will email you the information you require to complete the registration process online and create your patient account.

The practice may call you to confirm your details if any details entered on our form is different from what we hold on file.

To access this service once registered, please click on link below and input your username and password to log in - http://www.patient-services.co.uk

We are having some updates to our IT system which may affect you logging onto your On Line Prescription Service if you were previously a patient at Carden Medical Centre. Here is a guide on how to do this PFS - Unlinking relinking accounts.docx

If you have any further queries, please don't hesitate to get back in touch with the practice during normal working hours.

scriptOur timescale for preparing repeat prescriptions has been extended to 4 working days. All prescriptions will be sent to a nominated pharmacy.  Allow a further 2 working days before attending the pharmacy.

If you have not nominated a pharmacy, we will send your prescription to Anderson & Spence, Fountainhall Road.

Should you wish to change which pharmacy your prescription goes to, please contact the surgery to advise us of new details.

We also have a dedicated voicemail, which you can contact 24 hours a day.  Please only order for one patient during each phone call - phone again for the second patient.  Telephone 01224 562882 to leave a message - speak slowly stating:

  • Full name
  • Date of Birth
  • Address
  • Telephone number
  • Names of the medicines or appliances required

We ask that you request only those medicines that are listed on the right hand portion of your prescription.  These are the only items that have been approved by the Doctor.  In certain circumstances the Doctor may request that you be seen prior to prescribing any more medication.

On occasion you will also be invited for a medication review – this is simply to ensure that you are on the correct drugs for your condition and that you are not experiencing any problems and is a routine part of your care.

We can post your prescription to you if you provide us with a stamped addressed envelope.

Due to strict interpretation of prescribing regulations only GPs can phone prescriptions through to Pharmacies.  This will only be done in emergency situations.

Please ensure that you order repeat prescriptions at least one week in advance so that you do not run out of your medicines.
