To register as a new patient you can print off registration forms from the links at the bottom of this page.  If you do not have access to a computer or printer, please come in and collect paper forms from our Reception desk.  The completed registration paperwork can be sent by e-mail to gram.hamiltonclinical@nhs.scot or you can deliver to our Reception desk or post completed forms through our letterbox.  It can take around 5 working days for your registration to be processed once completed forms have been received.  If you are unsure whether your address is within the Practice boundary, please call us on the above number to confirm.

Forms required to register:-

For adults and children over age 6, you will need to complete 2 forms for each person:

1) Registration Form (GPR)

2) New Patient Questionnaire - there are different forms for adults and children 

For children age 6 and under, you will need to complete 3 forms for each child:

1) Registration Form (GPR)

2) New Patient Questionnaire - Child

3) Immunisation Form

If you are on medication please provide us with the re-order part of your prescription, if possible.

New patients may see our Healthcare Assistant for basic health screening, although this is not a requirement of registration with the Practice.
